Ambassador Club Parents,

Below is the waiver which our Club facility, Community Bible Church, requires every Parent and Student in Ambassador Club to read, agree to, and sign.

Click on the link below to download, print, and sign the CBC waiver. Once you've read and signed it, include it with your Ambassador Club Registration info from the TAB above and either mail these documents and your Club fees to our Treasurer and Registrar, Maribeth Helton, at 1229 Donelson Ave., Old Hickory, TN, 37138, or bring them to her at Club.
